Follow these steps to get the duplicate/triplicate copy of class 10th and 12th lost certificates.
Get a FIR/ general diary report from the nearest police station (It is free of cost).Download the application form from the undermentioned link http://cbse.nic.in/faq/dupform.pdfRead the above application form carefully. It contains details of your regional CBSE office and fees for providing duplicate/triplicate certificates.Take printout of the application form and fill it out very carefully.Visit your school to getSignature (principal) and stamp (School) on the application form.A letter from school on school letter head confirming that the said person has actually lost/destroyed the original certificate with principal signature and school stamp on it.If you don’t know your Roll number don’t worry your school has your record. Ask them they will provide it to you.Publish a notification in lost and found section in any national daily in the below mentioned format (FIR/General diary report is required for this).It is notified for the information that my original passing certificate of the main senior secondary examination of the year XXXX and Roll no. XXXXXXX issued by CBSE has been actually lost/destroyed. Name of the candidate- XXXXX XXXXX, Full Address/Tel.- Your address,(Ph.-XXXXXXXXXX).Prefer English newspaper.If you have photocopy of the lost certificates than it would be great but if you don’t have than don’t worry.Now contact your regional CBSE office and ask if anything other than above mentioned documents are required for the issue of duplicate/triplicate of the documents. (If they don’t pickup the phone don’t sweat it)You can also call CBSE helpline no. - 1800-11-8002 (TOLL FREE).Now you have two choices to get the duplicate/triplicate copy of the lost documentsVisit the regional office and pay the fees,collect receipt.
