                   Sub: Application for sick leave

Respected Madam,

       I am Ritwika Roy, student of class VII, section- B, suffering from high fever since last three days. My medical report is saying that it is dengue. So, doctor advised me to take rest for ten days.

         I would be highly obliged if you grant me the ten days leave from 10. 1.2020 to 20. 1. 2020. I have attached the medical report with the letter.
