See, you will have to write letters in many places in future too. Never forget basic format to write a letter. If it goes a little different then it's fine. But make sure to write neatly and represent what you want to say properly.

So, in general a formal letter consists of address to where and whom you are writing the letter,that is, to the principal and your school/college name. Then write your designation, that is, your name and the class and roll number(it varies depending on your institution) . These addresses should be written to the left.

Then don't forget to write date to the left.

First mention the subject you are going to write. In your case the subject is “Permission to not to wear shoes” .

The start the letter by introducing yourself. Mention your name, class and roll number. State why you are writing your letter. Then state why you need persmission.

Then mention to write “Please grant me permission.” . Then write your designation again to the right.

When you write clearly to the point, it is fine when your letter is small.
