I have a policy in your Company in the name of Nancy Sharma, Policy no. 52943/ 2009.

The nominee my policy is Mr. Pawan Sharma, my husband.

But now I want to add a new name as nominee-it is Reyansh Sharma, my son.

I have attached the necessary documents along with this letter.

Thus, I hereby request the concerned authorities to pay a heed to my request.
