With due respect and humble submission I beg to state that I, (your name) a student of Class-… of your school was absent for 5 days on account of my illness, i.e. I was suffering from high fever. I was said by doctor to rest in bed for minimum 4 days and to take the prescribed medicines.

I hope that you'll be kind enough to permit me and grant me leave of absence for those days.
