Respected Madam,

I am writing to request you to sanction leave for seven days starting from 12th July to 20th July. My Daughter has been diagnosed chicken pox and had to be admitted  in the hospital and needs to be taken care for further tests. It is infectious for other students so, doctor has suggested me that she will stay at hospital till recovery.I will report on school as soon as she will be discharged from the hospital.

Kindly consider my application and oblige me.
