1) Number of questions should be limited.
2) Questions should be simple and clear
3) Questions should be logically arranged
4) No personal questions should be asked or avoided i.e., questions which may hurt their religious feeling, emotions etc. should be avoided
5) No mathematical questions
6) Questions can be of open ended or close ended. Open ended provide more responses but are hard to interpret and hard to score due to lot of variation in answers. Close ended questions are two way questions (yes/no) or multiple choice questions. These are easy for statistical analysis
