All the solutions on the web are completely wrong. Because Athlete's Foot is not caused by protozoans - it is a fungal disease. And tuberculosis is not 100% not fungal. I can't believe 42 people upvoted the previous answer... Here is the answer, according to my viewpoint:

1. Chronic Disease is Cancer, which is a non-infectious disease

2. Congenital Disease is Diabetes, which leads to High BP

3. Acquired Disease can be an Infectious Disease, which matches to Tubercolosis

4. Fungi causes Skin Infections, An example of skin disease is Athlete's Foot
