At the time of the foundation of German Empire, Kingdom of Prussia was the largest part of the empire. At the end of world war II, poland was able to seize independence, and Germany was compelled to cede line of territories to it. The chunks that were ceded with Germany were those with Polish majority. 

The other parts of Poland were ceded with Czechosolovakia , France, and Poland.
