It is difficult to extrapolate from past wars, because nuclear weapons are so vastly different in the amounts of energy and effects than conventional weapons. In the aftermath of the Hiroshima and Nagasaki bombings, which were extensively studied along with the many nuclear tests conducted during the 40’s, 50’s and 60’s, most people as well as most leaders were sufficiently moved by the effects of the (by comparison) small weapons used in Japan and the potential effects of the huge effects (by comparison) of hydrogen weapons quickly developed and available that extreme efforts were made to prevent the future use of nuclear weapons. Much of that experience is now beyond the experience of those living today and in leadership positions.

Kennedy and Khrushchev had experienced WWII, and despite tangling themselves together in the Cuban Missile Crisis, both were sufficiently cowed by the possibilities of nuclear war that they paused and compromised. They then started down the road towards arms agreements that at least limited the arms race and also provided some means of negotiation (such as the red phone, publishing military movements and exercises, etc.)

It is difficult to answer your question, because the possible scenarios that might lead to WW III and the use of nuclear weapons and the ways those scenarios might develop are large and complicated. India - Pakistan, US - Russia, US - China, China - Russia, Russia - France/Britain are some possibilities.

I have read a great deal of nuclear holocaust fiction, starting with Alas Babylon by Pat Frank, written in the late 1950’s. One novel I recommend is The Third World War by Humphrey Hawksley. That novel is so frightening because you realize that there are many actors within governments that possess nuclear weapons that could cause an exchange, and once one weapon is used, the urge to retaliate will be high and the time for reason will be small. Kennedy waited almost 11 weeks from the time missiles were discovered in Cuba until the time he declared the quarantine. Nowadays, with the internet and social media, with our countries divided between left and right, would any leader be able to resist retaliating?
