Its an idiom. Both neem fruit and bittergourd(karela) are bitter. Here Ranga is talking about the type of girl he wants to marry. He says that one can't marry a girl who has milk stains on one side of her face (very young that she drinks Milk) and wetness on other (who cries). And one who doesn't even know how to bite her fingers. ( too young or an infant) Thus both these situations are worse for ranga to marry the girl. That is both these situations are bitter.

Answer:

"One a neem fruit and other a bitter gourd" is  a famous Hindi proverb which means both the situations are worse as both neem fruit and bitter gourd are equally bitter in taste.

Explanation:

The above proverb is taken from the story "Ranga's Marriage". When the narrator and Ranga were discussing the prospects of marriage, the narrator said the proverb "One a neem fruit and other a bitter gourd".

He wanted to find the right girl who would be a perfect match for himself. To him "right" girl meant the one who was neither very immature nor very old. According to Ranga, one shouldn't marry such a little girl who still had milk stains on her cheeks.

The narrator then said the above mentioned proverb in this context. According to the narrator, it was equally unfortunate to remain a bachelor for the rest of the life and to marry an immature girl just like both neem fruit and bitter gourd are equally bitter in taste.
