It is advised because the burning coal / Angethi produce harmful gases when they are burnt like carbon monoxide and argon. If they are played in closed room then it can kill the people's which are present there because the gases will take place of oxygen after some time which is present there. That's why it is advised to open the door.

it is said to keep an burning angethi outside of room becuse in winter the room surely will be closed and the burning coal will produce carbon monoxide which is harmful for human health and even if a peson not put the angethi outside the room in night and slept the person will be killed because of not getting oxygen in the room.
