a) 'Magnitude of u and v are same' means object is placed at centre of curvature.

In S. No. 3, object distance is -40cm

|u| = R = 40cm

=> f = R/2 = 40/2 = 20cm

b) In S. No. 6, u = -15 cm

Since f = 20cm, object is between focus and lens. In this case, the image formed is virtual and erect. The image is formed in the same side as object.
So v should be negative, but it's positive. So it is wrong.

c)
In S. No. 2, object is beyond 2f. So image will be formed between f and 2f in the other side. It will be real, inverted and diminished.
