Please tell how to create a survey on use of plastic in locality

We are a group of students from Raffles Institution (Junior College) comprising of Low Ying Xin, Clarissa Poh, Daniel Toh and Jethro Lin and we are conducting a study on public awareness and willingness to commit to using less plastic bags in their everyday life. This study is a research for our GCE A-level subject, Project Work.

The aim of the project is to understand the perception of the public on the abolishing of plastic bags in Singapore’s supermarkets. Your truthful response to this survey is of crucial importance to the outcome of the study.
