The question does not specify risks to the organ donor or organ recipient? So not sure how to answer.
But, let me give you the risks for both donor, recipient, and the whole transplant team-the surgeons/the evaluation group/nurses/transplant coordinators/etc/
For the donor, obviously none for cadavers but for living donors the big risks are just as for any surgical procedure - be it infections post-op, just ongoing life without one of your kidneys or part of your liver, the 4–6 week recovery period where you may experience lost wages, etc. The risk factor is generally pretty low for living donors-especially if we are talking about one of the primary transplant centers in the US-you want to utilize a center where they are doing lots of living donor transplants - preferably over 100 per year.
For recipients, sure lots of risks-from rejecting the organ to infections to just about anything; but look at it a different way; IF the recipient doesn’t get a transplant, life is will never improve, you will just get sicker and sicker, so the risk is more in not having the transplant.
Now, for the transplant group well each transplant they perform has risks, be it organ rejection by the patient, be it not arriving at the optimal post-transplant immunosuppressant regiment, be it a patient that contracts an infection that has to be treated, just all kinds of things. And, they never really know how the patient is going to do psychologically speaking, post-transplant. Will they take their meds, will they take care of their overall health, will they eat right, how will they do from an overall perspective, family support, etc.
