See, Class 11th is a difficult stage compared both to class 10th and 12th, You scored 90% in 10th has no connection with class 11th and 12th, Ohk, So read this very very carefully, ohk! Foget your last year, you want to become a doctor, now if you go to class 12th obviously your class 11th part is not clear which means that in class 12th you would first complete basics ( class 11th stuff ) and then class 12th and then boards and practicals and Entrances, so my advice for you is , REPEAT CLASS 11TH, it really doesn't matter if same teachers or your juniors sit with you in the same class. WHY REPEAT CLASS 11TH? Because, you can clear up your doubts, can double up your knowledge and eventually you gain lot of confidence and when you enter class 12th with so much confidence , everything would seem easy, trust me and also you would get more time for entrance preparation stuff. 
My advice is, if you go to class 12th now, without cleared base of physics, chemistry then obviously in the end, you have to drop 1 year for medical preparations. 
And if you are feeling akward staying in the same school, try other schools. The other schools would hesitate taking on a failed student but as you would be doing superb in your first terminals, they would be glad that you joined their school.
Dont be upset, Medical itself is a time taking process, think about getting into AIIMS, think about clearing Kinematics, Laws of Motion, Oscillations .. just imagine if these chapters become your strong hold and your favourite chapters. 
Repeat class 11th, seriously, next year you would be laughing ki yaar mai essa tha, itne kaam aate the, and a new level of confidence would return in You which you had in class 10th. Do well, Duniya ka kaam hai bakwaas karna.
