Typically it is formed through combustion. When hydrocarbons burn in oxygen they release CO2 and H2O. This is included in metabolic combustion too. When our cells burn carbohydrates it forms CO2 and H2O. Now there are a lot more complicated thins that happen in our body but that is part of it. Plants can also perform respiration. There are many ways that water is formed but most of the water on earth would have came here from our violent history. Water would have formed from hydrogen and oxygen in our galaxy's hot history. Then perhaps comets or asteroids carrying this water brought it to us. The intense heat from Earth could have also combined hydrogen and oxygen that was present on our planet.
