Shift due to glass slab:

 t ( 1 - 1/μ) = 6 ( 1 - 2/3) = 6 (1/3) = 2 cm

Since radius of curvature  = 40 cm

Object  must be placed on a distance of  42 cm

so that due to shift the image and object will be at 40 cm and both coincide.

Hence the distance of the object from the mirror is 40 cm.
