Sometimes woman may deliver more than 2 offsprings at a time. Why does this happens? Full explanation needed

It can happen when two ova leave the ovaries on the same time and get fertilized by sperm cells.
Again, it happens when one ovum, after getting fertilized splits into two parts.
The first case results in unlike twins while the other causes like twins.
