Reaction of sodium dihydrogen orthophosphate and sodium hydrogen orthophosphate to form sodium triphosphate is as follows -

NaH2PO4 + 2 Na2HPO4 --> Na5P3O10 + 2 H2O

As we can see stoichiometric ratio of reactants is

NaH2PO4 : Na2HPO4 = 1 : 2 = 1.5 : 3
