Bringing a reversible change in milk:
Heating the milk: When we heat the milk it can again be brought to the normal temperature. So heating milk is considered as an easy example of reversible change.
Bringing an irreversible change in milk:
Transformation if milk to curd: The transformation of milk to curd is considered as an irreversible change as once a little curd is added to it, I cannot be brought back to milk again. The change happens on a chemical basis, which is irreversible.

As the name clarifies, its a sort of change which can be reversed back to the original no matter how many times you do that.

Reversible change in milk:-
When the milk is kept in refrigerator it freezes. Now the milk can be brought to its previous state by heating. So freezing of milk is a reversible change because there is no change in the chemical composition.

Formation of curd or dahi from milk is a irreversible change. Due to the lactobacillus bacteria the curd formation takes place . Its formaion is irreversable & there is a change in the chemical composition because once the curd is formed , we cannot seperate the substances from it.
