In the case of teachers failing learners by not giving them the proper education required to help them, this means that teachers have 'failed' and disappointed us (the learners) and because of that, as learners we lack the skills and information needed to be successful in life (e.g. get into a good college and get a well-paying job).

In the sense that teachers fail us by granting learners low marks and grades, this is connected to the first definition of fail - because a reason why learners get low marks is because they haven't been properly educated - which is the teacher's job. A job that they have failed to do.
