One reason tests don't necessrily reflect ability is “test anxiety”. You can know the material completely but if you are afraid or suffer from anxiety, your mind can go blank. Another reason is that some abilities simply cannot be

Another reason is that some abilities simply cannot be tested.

Also if you have a vision or auditory impairment that is not known or has been treated improperly, the test taker will not be able to use their knowledge as well.

Too little sleep, being hungry, an argument, or an illness, etc. are all reasons the test taker might not be at their best when taking the test.
