Coordination complexes are formed by the bonding between metal ion and ligand. Here ligand is the atom or ion or molecule that can give its extra pair of electrons to form the coordinate bond. A coordination complex is always composed of complex ion and counter ion. Here counter ion is always outside the square bracket.  

Hence:

\bold{\left[{Pt}\left(\mathrm{NH}_{3}\right)_{6}\right] \mathrm{Cl}_{4} \rightarrow \{Pt}\left(\mathrm{NH}_{3}\right)_{6}^{+4}+4 \mathrm{Cl}^{-}}

Therefore it will form total 5 ions; one complex ion and four Cl- ions.
