Well, First off, This is not a 3D shape. But, I assume that you mean this net is to be folded to form a 3D shape. In that case, It will be a cube.

Each of it's side will be of 12cm. And, the surface area of a cube is given by,

S.A=6a^2, where a is a side of the cube. Since a=12, the answer will be,

6 x 12 x 12=864cm^2

Hence, your answers are, 1) cube and 2) 864 cm square
