Open the Annotation Symbol family.

Click File menu > Import/Link > Image in versions prior to 2010 or go to the Insert Tab and select Images in 2010 and later verisons. With any image family, it is recommended that you add reference lines around the image to aid in selection once the image is nested.  

After the image has been imported, click Settings menu > Family Category and Parameters in 2009 and earlier versions, in 2010 and later, select Family Category and Parameters on the Home Tab. Select the Shared check box. This will make the image visible when you load the family into a project.

Create a new Annotation Symbol family. The previously created Annotation Symbol family will be loaded into this new family. Load the Annotation Symbol family containing the image into this new family.

In the new family, create the Visibility parameter by clicking Family Types > Parameters > Add. Enter a name, change Type of Parameter to Yes/No, and specify Instance or Type.

Select the nested Annotation Symbol Family and then click Element Properties on the Options bar.  

For the Visible parameter, click the Associate Family Parameter button.  

Select the Yes/No Visibility parameter. The Visibility will now be linked.

Load the family into the project.
