Subject: Application to allow my child to attend School

Respected Madam,

With due respect, I would like to inform you that my ward _______________________, studying in

class ________________ is attending online classes conducted by School. As per the recent govt.

guidelines, I request you to kindly allow my ward to come to school to attend classes.

I assure you the following:-

1. My ward or any of the family members are not suffering from any symptoms like fever,

cough, cold, breathlessness, sore throat etc.

2. We have not travelled domestically or internationally in last 20 days.

3. We have not come in contact with any covid-19 cases in last 20 days.

4. My ward will wear mask at all times and will maintain social distance.

5. He/She will follow all rules and regulations imposed by the school as seen necessary.

6. Transport facility will be provided only on short routes after receiving consent from

parents.

We understand that school and Management is taking all necessary safety and hygiene

precautions and it can not be held liable in any event, what so ever.
