Just to let you know, Everyone's perspective is different, every situation is circumstantial and every job is better in its own way.
Below views are just my thinking and it might not resonate with what others think or their perspective of the reality. I will go with the engineering as I don't know much about Medical Entries in armed forces.
You can be in the Armed Force and be an Engineer at the same time. There are multiple entries for that, like TGC(PC), SSC tech in Army, Navy Entries(SSC) and technical entries in Air Force via AFCAT gor both PC and SSC.
Goverment invests heavily on their engineers. You learn from the best and they encourage you to take up further specialization and higher studies. There you get real life hands on experiences which are seldom money/ business driven.
Apart from these stuffs, you learn many life skills which will enhance your personality and communcation ability. You will be more professional in your conducts than your normal civil counterparts. You will live much more disciplined life. If you have 'IT' in you, you will experience a good, fun, adventurous and exiting life on an “Auto-Pilot” mode.
Coming to normal engineering life, if you get into some Goverment PSU then its good. It will be a good life and it will be upto you, where you take it from there to make it exiting and full of fun. Even if you just perform normally, you will live a good life.
If you get into the private engineering jobs, lets say in information technology, then you are in the driver's seat with number of variables affecting your life. What you do in your job impacts your life. Remember, there is no Government to protect you. You have to keep up with latest trends on technologies. Long working hours and sleep takes up most of yourday and you are left with very little time to do anything else. Everything in your job will be money/ business driven.
