The answers on basis of my research and knowledge are as follows:

(i) The river water is most polluted water as most of the wastes whether domestic or industrial are discharged into rivers on large scale.

(ii) Industrialization is major source of water pollution. The industries generate chemical wastes, which are very toxic, on a large scale and release them directly into water which becomes major source of water pollution.

(iii) The water pollutants may be pathogens, organic and inorganic wastes.

(iv) BOD means biochemical oxygen demand. It is defined as amount of dissolved oxygen needed by aerobic biological organisms to break down organic material present in a given water sample at certain temperature over a specific time period.

(v) The excess of fluorine in drinking water causes fluorisis which affects bones and teeth of human beings.

(vi) Chlorination is done to water to kill bacteria and other pathogens which can cause diseases to human beings.
