Basically DILI is a diagnosis of exclusion [of other causes of liver disease like viral, autoimmune, metabolic] but it has to be supplemented by sound clinical history of host , environmental and drug related factors, physical and lab examination including liver biopsy is absolutely indicated. These are assessed together for probability by different scales e.g. RUCAM scale. But the most definite proof is of course positive rechallenge
