A good questionnaire shouldn't be complicated.  

It shouldn't be too lengthy or difficult to answer.

It should have sensible questions and shouldn't be biased (there shouldn't be any negative question about a particular community)

The questions should be designed in a way that you get your desired result easily after the survey.

It should be well thought out, logical and should clearly reflect your objectives.
