What causes a leap year?

when the earth rotates in its orbit it makes 365 days and 5. 54 hours(round figure 6 hours) and continuing the same, after four years it makes a day,due to which we celebrate leap year in every four years................
question raises that then that don't we follow the real one( the 365&5hours 54 min)...............if we follow the first one then we won't be able to make a perfect year.......That's why....
