hey here is your answer

Changing the amount of heat energy in a substance usually causes a temperature change. However, during a phase change, the temperature stays the same even though the heat energychanges. This energy is going intochanging the phase and not into raising the temperature.

Heya.....

See here for your answer....

==============

Phase chances can be the physical changes just like changing of liquid into solid or in the gaseous form...

This can be occur due to...

√ Change in heat level....

Suppose,, if we have to change the phase of liquid , so we should first stop the heat getting to it, and coiled it or to give it more heat .....

This creates a sensation in the molecules and responsible to change in the phase...
