Hierarchical bounds.

Poor knowledge on technology being used.

Poor disaster management and control by the govt.

Explanation:

Imagine this, it wouldn't have been a disaster if on knowing about the xenon pit in the reactor core, the supervisor would have asked the controller to shut down the reactor for next 24 hours.

The recklessness of the supervisor who envisioned of a successful test run at all cost led to the point of destruction.

It would have been averted if the control rods have been slowly lowered down rather than all going into the reactor at the same time. Since they didn't know anything about graphite tips on the control rod which acts as moderator in the RBMK reactor.

All this happened because state kept it a secret from the people working there and lied to them all for what? it was cheaper and high power producer reactor unlike western reactors.
