what happens to the respiratory system of a pilot who is in power to drive a supersonic plane

Nothing. Fighters are pressurized by an isobaric/differential(altitude-6 divided by two.) so there will be some ear popping but no problem in the respiratory system.

Due to lack of O2, the lungs will start breathing anaerobically i.e.; without
oxygen. This may lead to cramps.
