Chemical Formula of Calcium Carbonate is CaCO3.

On heating Calcium Carbonate decomposes into Quick Lime and Carbon dioxide gas

CaCO3(aq)=========>CaO(s)+CO2(g)

The Reaction is endothermic in nature

Does Decomposition only occur on heating?

No, decomposition reaction can also occur on passing electricity or due to sunlight. They are called Electrolytic Decomposition Reactions and Photochemical Reactions respectively
