These days the rate of technology adoption is speeding up but there is still a delay, particularly for tech that requires a significant infrastructure change to support it. As others have pointed out, small towns and rural areas have been slow to get internet service and cell phone coverage, the best hospitals with the newest methods tend to be in more populated areas with rural areas being lucky if they even have a clinic, as self driving cars are rolled out they’ll only be in urban areas to begin with, etc.
