Drug adulteration is the act of adding one or more substances to drugs of abuse. Street drugs are produced in substandard laboratory conditions and they deem to be adulterated intentionally or inadvertently by drug producers and dealers. There are some reasons for adulterating street drugs; it is commonly done to combine cheaper drugs with drugs of interest to change the physicochemical or psychological activity of street drug or bulking up the final product with inert substances such as talk, milk powder, glass beads and etc. Adulterated drugs can be an important health hazard and the dangers of these drugs are hidden for abusers. Therefore healthcare providers are responsible to report issues and outbreaks of adulterated drugs. Methods: There are some formal and informal reports regarding trends and constituents of adulterated drugs in Iran. In previous studies heroin, methamphetamine, crude opium and Iranian crack were analysed in Legal Medicine Research Center, Forensic Toxicology Department, Tehran, Iran fore the detection of active pharmaceutical ingredients and microbial contamination. Results: Tested drugs were adulterated with Bacillus spp., psychoactive drugs such as tramadol, methadone, dextromethorphan, phenobarbital, diazepam and even lead as a heavy metal.
