The term arose from the book of Genesis in the bible. Adam was the first man and lived with Eve (the first woman) in the Garden of Eden. Eve disobeyed God’s orders not to eat the fruit of the Tree of Knowledge of Good and Evil, and gave some of the fruit to Adam.

Although Genesis doesn't say what kind of fruit it was, since mediaeval times legend postulates that it was an apple. The legend also says that the first bite stuck in Adam’s throat and the chunk of apple is present in all his male descendants.

This “explains” why the Adam’s apple is more noticeable in men, (though exactly the same structure is present in women).
