nuclear fusion is a reaction in which two or more atomic nuclei come close enough to form one or more different atomic nuclei and subatomic particles (neutrons and/or protons). large amounts of energy is released in this reaction.

First, the word 'fusion' means to combine.
So , nuclear fusion is the process in which two nuclei of a light element combine to form a heavy nucleus.

Best example is that of hydrogen and helium. Two nuclei of hydrogen combine to form heavy nucleus of helium element.
Conditions to carry out nuclear fusion are :-
1) Millions of degrees of temperature
2) Millions of pascals of pressure.
