A method, suitable for the clinical laboratory, is described for the estimation of the total and direct reacting bilirubin in serum. Hemolysis in the serum specimen is without influence on the estimation of normal bilirubin concentrations, but affects the determination when the serum bilirubin is elevated. The direct reaction is estimated at 5 and/or 30 min. The 30 min value correlates with the coupling of the conjugated bilirubins and, when calculated by difference, is indicative of the unconjugated bilirubin content of the serum. The indirect reacting bilirubin fraction is not or only slightly increased in blood serums, obtained from patients with regurgitation jaundice, which indicates no distinct elevations of the unconjugated bilirubin in this condition.
