Leave a line
Date in the form 21st August, 2017 (write the date when u're writing the letter)
Salutation
1st para: asking about health, family. No hi, hello.
2nd and 3rd: whatever the letter is about
Last line: “convey my regards to your family”, “convey my regards to grandpa”,”I'll be waiting to meet you”.
Yours lovingly,

P.S.: in formal letter, the subject may or may not be mentioned. You won't be penalised.
All the best!!
Edit 1: It seems some schools penalise if subject is not mentioned in formal letters. So, I advise you to clarify it with your teacher. I was told by my teacher that subject is not necessary. Sorry for the inconvenience.
