In a mercury thermometer, it prevents the mercury from dropping immediately back into the bulb when the thermometer is removed from the patient. That constriction may or may not be present in an alcohol thermometer

The constriction enables a practitioner to take a temperature reading without the temperature going down once it is removed. Some practitioners are short sighted and if the thermometer had to be read whilst it was in the patient it would be awkward at best, especially if rectal temperature is being taken.
With the constriction, once removed, the thermometer will show the maximum temperature.
If you vigorously shake the thermometer the thermometer is reset
