Chromosomes are structures within the nuclei of eukaryotic cells that contain deoxyribonucleic acid (DNA) combined with proteins. Chromatin refers to the material of the chromosomes – DNA plus proteins. ... In mitotic chromosomes, scaffold proteins fold the chromatin fibre into loops along its length.

Chromatin is like a thread like structure, which are found throughout interphase. The DNA are packed loosely.
But in Chromosomes DNA are packed tightly, which are found only during cell division.
When a cell is going to divide it coils with Chromatin to form Chromosomes.
Chromosomes are easy to divide than Chromatin.
In Chromosomes DNA is used for macromolecule synthesis while in Chromatin DNA is not used for macromolecules synthesis.
