Okay, to understand this concept of worm hole you have to first understand the nature of space, that it is not flat. Gravity has the ability to bend and stretch the fabric of space-time.
Worm hole is something hypothetical. It exists in theories. It's a way through which one could travel the distance of millions and billions lightyears within few days or hours depending upon how much space is stretched and the distance of wormhole.
But in any condition it's gonna provide us shortest way between any two much too far distances.
Take an example of crossing a mountain to go to the other side and you have only one way that is to climb it up and then back down on the other side but not if there was a giant wormhole that cut horizontally through the rock. Hope, this example gives you an idea of a wormhole.
Does it exists???
Hmmm... Yes but only in theories, we lack technology and that precise engineering.
But this idea wasn't an invention of science fiction writers.
In 1935, Albert Einstein and Nathan Rosen wrote a paper in which they showed that general relativity allowed them to form what they called as bridges. It's exactly what today we call as wormholes.
But they also said that they didn't lasted long enough for a spaceship to cross but it was also suggested that advanced species can keep those bridges open.
Theoretically, Yes time travel is possible through it.
As a physicist has written that small wormholes could join two places which are not spereated by any distance. Which means your past and future.
