If it is a short story, you may begin with the introduction of the current situation, and move on with the introduction of the characters in brief.
Never start with " Once upon a time..." that's childish!
You may start with the background of the story such as the location or the general nature , where the story is based,etc.
If it says...write a story beginning with the words... begin accordingly without wasting time for introductions on required.
At that point of time, The Introduction may be the current situation according to the topic. For example: If I had known how to drive the car, I could have escaped the situation....where "If I had known " is the topic.
