It is hard to know where to start, because the question doesn’t state whether a particular country’s rivers are the subject of the enquiry, or rivers in general. In the UK the state of the river systems have been improving significantly over the past 30 years or so, and salmon are now seen making their way up some major rivers to spawn, where once those rivers would have been a lethal barrier to them.

Sewage from cities, towns and villages must be treated before discharging to rivers, and it is the job of the Environment Agency to ensure that the operators of the sewage treatment works (the Water Companies) maintain their works so that the effluent meets certain quality standards that allow the river to complete the purification process without detriment to the fish, invertebrates, and plant life, and to allow abstraction downstream to further treat to potable water standards.

It is also incumbent on the Water Companies to impose controls on the industrial effluents discharged to sewerage systems so that the biological processes of the sewage treatment works are not harmed by toxic constituents of the industrial effluents
