  • Concentrate on the Root Cause(s) rather than the Symptoms

When attempting to improve performance, it is very easy to focus on the symptoms that are most visible in the organisation. Unfortunately, yelling "Stop being late with deliveries!" will not solve a delivery problem; if it did, every executive would have resolved the issue by now. Instead, we must ask "Why?" and drill down until we find the core root causes.

  • Disable All Root Causes

If we only address one root cause of a problem with multiple causes, the problem will resurface quickly and may change slightly enough to be considered a new or different problem. We should identify all root causes during our root cause analysis.

  • Don't confuse containment with resolution

As soon as an operational issue is identified, it should be addressed. Awareness should be raised, and those affected should collaborate to develop a short-term strategy to keep the problem from spreading beyond the immediate area while permanent corrective actions are developed and verified.

Examine Problem-Solving Outcomes

When a problem is certified as "solved" within the operation, it should be followed by a periodic and scheduled assessment of resolution to ensure that it is truly fixed. A post-mortem should also be performed to determine why the things that worked properly were effective and why the things that failed were ineffective.
