This is spite of being terminally ill and keeping it a secret. Anne remembered her grandmother as being loyal and good and having never let them down. She always stood up for her granddaughter no matter how badly she misbehaved. She wonders if her grandmother loved her or if she did not understand her either. She conjectures that her grandmother must have felt quite lonely in spite of being surrounded by people who loved her and that was something that happened all the time, to feel lonely even when one is loved by many people. Anne's grandmother would appear to her as a guardian angel, it was such a vivid image that she could even make out her soft skin of crinkly velvet.
