When the space shuttle reentered earth's atmosphere,The space shuttle had disintegrated.
The reason being a piece of foam insulation broke off from the Space Shuttle external tank and struck the left wing of the orbiter.This had happened in other launches but this was much more serious.There was no way it could have been prevented once the shuttle had been launched
